By completing this form, you authorize our veterinary team to provide the best possible treatment for your pet, working closely with your designated agent and within any cost limits you specify.

How to use this form:
- Fill out and sign the form. Also include information about medications and health concerns for each pet.
- Provide a copy of the form and your pet’s health information to your designated Acting Agent (the person/facility caring for your pets in your absence).
- Arrange a valid payment method with your Acting Agent. If you leave your card number with them, ensure they also have a billing name, address, zip code, 3 digit code, and expiration date.
- Keep a copy for your own records.
